Uninsured (non-OHIP covered) Services Policy

OHIP/Provincial healthcare does not pay for all services that you request from your doctor(s). Services that OHIP does not pay for are called “non-insured or uninsured services” and it is illegal and fraudulent for doctors to bill OHIP for them. In order to account for costs associated with these services and ensure prompt service, it is necessary and legal for our practice to charge for these services.

Every effort has been made to account for most of the community requested services in the Charges for Uninsured Services document. If the service requested is not listed, please communicate that to the office staff for further clarification. The fees are based on the Ontario Medical Association suggested fees as found in the 2018 EDITION of the OMA Physician’s Guide to Uninsured Services.

All uninsured services must be paid in full when rendered. You have the right to a receipt and the office staff will provide you with one upon settlement of your fees. We accept cheques and cash.

Please Note:

There are a number of EXEMPTIONS when charging for the completion of a third-party report form. The following list contains some of the common forms physicians are NOT permitted to charge for its completion:

  • Application for Accessible Parking Permit

  • Transit forms for a disability

  • Permanent Residence Cards

  • Request for Birth Certificate Forms

  • Children’s Aid Society Forms (on behalf of child)

  • Canada Passport Application
